HarmonyCares seeks a PRN Family Nurse Practitioner to deliver home-based primary care to medically complex, home-limited patients. In this mission-driven role, you will conduct in-home visits, perform comprehensive assessments, manage chronic conditions, and coordinate care using both in-person and telehealth approaches. You’ll collaborate closely with an interdisciplinary team to reduce avoidable hospitalizations, improve outcomes, and support patients and caregivers through education, care planning, and compassionate, patient-first service.
