Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Prior outpatient orthopedic experience.
• New graduates are not accepted.

Assignment Details

• Schedule structure: 5x8s or 4x10s.
• Evening coverage is required at least 2 nights per week.
• Clinic hours are Monday–Thursday 6:00 AM–8:00 PM and Friday 6:00 AM–6:30 PM.
• Floating requirements: No.

Location Highlights

Fox Lake offers access to Chain O'Lakes State Park and the Chain O'Lakes waterway, with boating, fishing, and trail options throughout the area. Fox Lake sits about 15 miles from the Wisconsin border and approximately 60 miles northwest of downtown Chicago, which supports both outdoor recreation and access to a major metro area. The village is known for lakefront living and proximity to Volo Museum and Six Flags Great America in Gurnee, both within a reasonable drive for day trips. Commuters and travelers can also use the Fox Lake Metra station on the Milwaukee District North Line for rail access toward Chicago.
