How you’ll make an impact in this role

  • Enhanced Communication: RN Patient Navigators serve as a central point of contact, ensuring that patients and families clearly understand complex diagnoses and treatment plans.
  • Care Coordination: They facilitate seamless transitions from the ICU to step-down units or to home, reducing treatment delays and ensuring continuity of care.
  • Barrier Removal: By identifying and addressing obstacles such as financial, transportation, or social issues, they ensure timely access to necessary services.
  • Reduced Readmissions: Providing follow-up support and education on self-management helps decrease emergency room visits and urgent readmissions.
  • Emotional Support: They provide psychosocial support to patients and their families, helping them navigate the distress associated with critical illness.

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • Licensed Registered Nurse obtained prior to hire date or job transfer date required.
  • Registered Nurse credentialed from the Oklahoma Board of Nursing required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What additional preferences we're seeking

  • 5+ years of Critical Care nursing experience, preferably in an ICU setting
