Home Health Speech-Language Pathologist

Intermountain Health is seeking a Homecare Speech Language Pathologist to provide high-quality, patient-centered speech therapy services in patients’ homes. In this role, you will assess, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders across diverse adult and geriatric populations. Responsibilities include developing individualized treatment plans, providing evidence-based interventions, educating patients and caregivers, and collaborating closely with an interdisciplinary homecare team. You will document care in the EMR, contribute to quality improvement, and support Intermountain’s mission of compassionate, affordable community care.
