Clinical Pharmacist I

In this patient-first, mission-driven environment, you will verify and dispense medications, perform clinical medication reviews, and collaborate closely with physicians and nurses to optimize pharmacotherapy. Responsibilities include medication reconciliation, therapeutic drug monitoring, antimicrobial stewardship support, and patient counseling on medication use and adherence. You will document clinical interventions in the electronic health record, contribute to quality and safety initiatives, and participate in education for patients, staff, and learners.
