Job Summary:

As a case manager, you will make timely, independent decisions to support children and families with medical, educational, emotional, and social services. You will coordinate critical resources, ensure 24/7 emergency availability, lead family meetings, and guide families toward long-term permanency goals. This role involves regular in-home visits, close collaboration with caregivers, courts, and community partners, and a strong commitment to documentation and outcome tracking. Transportation, legal support, and attending court are also essential duties. Ultimately, you help ensure each child's safety, stability, and long-term success.

This position is more than just a job—it's a commitment to walk beside children and families in their most vulnerable moments. Your compassion, consistency, and courage can help a child feel seen, a family feel supported, and a future feel possible. Every visit, every plan, every court hearing matters—because behind each task is a life you're helping to shape with care, dignity, and hope. If your heart beats for change, healing, and purpose, this is where your impact begins.

Qualifications:

  • Bachelor's degree in social work, counseling, psychology, education, sociology, or criminal justice.
  • Minimum of one year of professional employment in the delivery of protective social services, family preservation services, or targeted case management services in a public or private agency.
  • Demonstrated experience as a positive contributor in a close team and community relationship, and with community resource mobilization.
