Hebrew SeniorLife, a Boston-based nonprofit senior care organization affiliated with Harvard Medical School, seeks a compassionate Nurse Care Manager. In this role, you will coordinate and deliver person-centered nursing care for older adults across senior living, skilled nursing, rehabilitation, home- and community-based services, memory care, and hospice. You’ll complete assessments, develop and update care plans, manage chronic conditions, teach patients and families, and collaborate with an interdisciplinary team in a respectful, inclusive, learning-focused culture dedicated to innovation in aging and elder care.
