CarePartners is seeking a Licensed Practical Nurse to deliver compassionate, skilled nursing care to patients in their homes. In this field-based role, you’ll perform assessments, administer medications, provide wound and catheter care, monitor vital signs, and document in the EMR. You’ll collaborate closely with RNs, therapists, physicians, and families to support individualized care plans that promote safety, independence, and recovery.
