Join our team as a PACE Primary Care Physician and help older adults live independently while receiving high-quality, all-inclusive care. What You'll Do - Serve as primary care physician for PACE participants (55) - Provide preventative, acute, and chronic care with a focus on aging with dignity - Collaborate with the Interdisciplinary team to coordinate holistic care plans.
