Behavioral Health Specialist

Oak Street Health is seeking a Behavioral Health Specialist to join our Healthcare & Medical Services team. In this role, you will provide clinical behavioral health services within a compassionate, team-based primary care model for adults on Medicare in underserved communities. Responsibilities include conducting assessments, developing treatment plans, delivering individual and group therapy, and collaborating closely with primary care providers and care teams.
