CenterWell Home Health is seeking an Occupational Therapist to deliver compassionate, in-home therapy services to seniors and adults with chronic conditions. In this role, you will perform comprehensive evaluations, develop individualized treatment plans, and help patients improve function, safety, and independence in daily activities. You’ll collaborate closely with an interdisciplinary team, educate families on home safety and adaptive strategies, and document care using our electronic health record.
