Adventist Health is seeking a Social Services Practitioner to support whole-person care for patients and families in our faith-based, mission-driven environment. In this role, you will perform psychosocial assessments, develop care and discharge plans, and provide counseling, crisis intervention, and resource navigation. You will connect patients with community services, advocate for vulnerable populations, and collaborate closely with nurses, physicians, and chaplains.
