Compensation: $19.00 - $21.00.
The base compensation range for this role is fixed, with a maximum cap of $21.00. We want to be transparent about this as we continue discussions.
Hours: Monday-Friday, 8AM-4:30PM

Graham Healthcare Group is hiring a Journey Administrative Specialist to join our dynamic team. Journey Administrative Specialist responsibilities include the day-to-day operations of Hospice's face-to-face needs. Support/assist Hospice team members to ensure face-to-face visits are assigned appropriately and completed on time. Assist the Hospice Journey team with needs if they arise. This position can be remote, but must be based near our Troy, MI, Mechanicsburg, PA, or Warrendale, PA office to be considered.
Journey Administrative Specialist Essential Duties and Responsibilities
Develop and process journey referrals while ensuring accurate documentation and workflow processes
Manage chart closure processes and provide documentation as requested
Work within various communication channels, including Teams, emails, HCHB, referral portals, SalesForce, and Medalogix
Assist other Journey team members with telephonic patient contact across multiple branches as requested
Demonstrate excellent customer service, both internally and externally.
Understand and effectively communicate HIPAA compliance, corporate compliance, and client confidentiality.
Communicate and collaborate with other Journey Team members, Palliative, Home Health, and Hospice teams as needed for internal team collaboration
Journey Administrative Specialist Qualification Requirements

High School diploma/GED
Previous Journey Counselor experience or Hospice related required
Medical Terminology knowledge required
Active listening skills
Customer service skills
Possess patience, dependability, good and sound judgement, and ability to manage concurrent demands
Proficient in Microsoft office suite including teams
Detail Oriented with use of time management techniques and tools
Certificates, Licenses, Registrations
Must have and maintain in good standing professional license, certificate, or registration, as applicable.
