Minimum Requirements
- Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Indiana Physical Therapy license required prior to start of assignment.
- BLS certification from AHA or American Red Cross may be required.
- Outpatient clinic experience preferred, including evaluation and plan of care development for musculoskeletal and post-operative patients.
- Experience with documentation, goal writing, letters of medical necessity, and charge entry in outpatient EMR systems preferred.
- Demonstrated ability to supervise and delegate to therapy assistants and students preferred.
- Maintain professional licensure and complete state-mandated continuing education as required by Indiana Practice Act.
Assignment Details
- Facility Type: Outpatient clinic.
- Shift Pattern: 5x8 schedule.
- Assignment Duration: 13 week assignment.
- Positions Available: 1 position.
- Candidate responsibilities include evaluation, development and modification of treatment plans, documentation in accordance with payer and clinic policies, and supervising discipline-specific therapy services.
