Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Home health experience
• Experience with EMAR system HCHB
• PT Practice 12365

Assignment Details

• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekend and on-call requirement: None
• Coverage areas: Marion and Hamilton
• Mileage reimbursed between visits from first patient to last patient
• Responsibilities include resumption of care, start of care, discharges, and oversight of PTA
• Productivity expectation: 30 points
