Full job description

As a Home Health Occupational Therapist, you provide evidence-based OT services to adult patients in their homes. You complete functional assessments, create and adjust care plans, and deliver one-on-one interventions to improve safety and independence. You educate patients and caregivers, coordinate with an interdisciplinary team, maintain accurate EMR documentation, and manage your caseload and travel within an assigned territory.
