Clinical Care Manager

As a Clinical Care Manager at VitalCaring, you will coordinate and oversee home health and hospice services to ensure safe, compassionate, and clinically excellent care in patients’ homes. You’ll manage a caseload, guide field clinicians, and collaborate with physicians and the interdisciplinary team to develop and adjust care plans. You will monitor outcomes, support regulatory compliance, and participate in case conferences and quality initiatives.
