Active Illinois Physical Therapy license required to start the assignment. BLS (Basic Life Support) certification from AHA or ARC may be required. Experience working in a skilled nursing facility or long-term care setting preferred. Demonstrated competency evaluating and treating geriatric populations, including fall risk assessment and restorative therapy preferred. Experience with documentation in electronic medical record systems preferred.
