Full job description

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.


Licensed Clinical Social Worker (LCSW)

Location: Rainbow Park Adult Day Health | Owings Mills, MD
Schedule: Part-Time | 16 Hours Per Week
Wage: $32.00/hour
Language Preference: Russian or Gujarati Speaking Preferred

Join our Adult Day Health Program as a Licensed Clinical Social Worker (LCSW), providing psychosocial support, counseling, advocacy, and care coordination to participants and their families. Collaborate with an interdisciplinary team to promote participant well-being, independence, and quality of life.

What You’ll Do

  • Complete psychosocial assessments and assist in developing individualized care plans
  • Provide psychosocial support, counseling, advocacy, and care coordination
  • Connect participants and families with appropriate resources and services
  • Monitor participant progress and collaborate with the interdisciplinary care team
  • Complete required documentation and participate in team meetings
  • Support participant well-being, independence, and quality of life

Education & Qualifications

  • MSW from an accredited program required
  • Active Maryland Social Work License required
  • Strong communication, organizational, and critical-thinking skills
  • Experience working with older adults, individuals with disabilities, or in healthcare settings preferred
  • Ability to collaborate effectively with program staff and interdisciplinary team members
  • Russian or Gujarati speaking preferred
