Full job description

Position Highlights

Full-time or part-time opportunities available
Round on approximately patients per day
Practice in Skilled Nursing Facilities (SNFs) and Assisted Living communities
Build lasting relationships with patients and facility teams
Work independently with a high level of clinical autonomy

Compensation

Base salary of $275,000
Productivity incentives with earning potential of $350,000 $400,000+ annually through RVUs

Ideal Candidate

Board Certified or Board Eligible in Family Medicine , Internal Medicine , or Geriatric Medicine
Comfortable practicing independently in post-acute care settings
Strong interpersonal skills with a patient-centered approach
Interested in developing long-term relationships within the facilities they serve
