Contract | Days | Outpatient Setting Key Responsibilities

Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction Conduct 60-minute evaluations and 30-minute follow-up sessions in an outpatient setting Maintain accurate, timely, and compliant medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities Delegate and direct discipline-specific therapy services for assigned patients Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations Required Qualifications

Active and unrestricted Physical Therapist licensure in good standing in the state of Colorado Adherence to state-specific Physical Therapy Practice Act and all applicable regulatory agency requirements Ability to complete documentation in compliance with state, federal, and intermediary guidelines Commitment to patient and employee safety standards including safe patient handling and infection control practices Completion of all state-mandated continuing education requirements Preferred Experience / Skills

Prior experience in an outpatient physical therapy setting Experience supervising or mentoring physical therapy students Familiarity with performance improvement methodologies and quality of care initiatives Why Join Us
