Lead Case Management Registered Nurse

TriStar Health seeks a Lead Case Manager RN to guide complex patient care coordination across our regional network. In this role, you’ll conduct clinical assessments, develop individualized discharge and care plans, and collaborate with physicians, nurses, social workers, and community partners to ensure safe transitions and reduce readmissions.
