BJC HealthCare seeks an Admissions Hospice Nurse to coordinate compassionate end-of-life care for patients entering hospice services in the St. Louis region. In this role, you will assess eligibility, complete admissions, educate patients and families, and collaborate closely with physicians, social workers, and the interdisciplinary hospice team. You’ll ensure timely, patient-centered transitions, support symptom management plans, and document accurately in the EMR.
