Medical Social Worker II

Kaiser Permanente seeks a Medical Social Worker III (LCSW) to provide advanced clinical social work services within our Healthcare & Medical Services department. In this role, you will conduct psychosocial assessments, develop and implement care plans, deliver individual, family, and group counseling, and coordinate resources for patients with complex medical and social needs. You will collaborate closely with physicians, nurses, and interdisciplinary teams to support safe discharges, chronic disease management, and behavioral health integration. This position emphasizes evidence-based practice, cultural competence, patient advocacy, and ongoing quality improvement in a fast-paced healthcare setting.
