Minimum Requirements

Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA Active State License is Required to Start the Assignment BLS Certification May Be Required from AHA or ARC Current unencumbered Physical Therapy license for the state of Indiana Recent home care experience proficiency HomeCare HomeBase proficiency Understanding of Medicare guidelines for home care CPR certification Driver's license, vehicle insurance, and dependable vehicle

Assignment Details

Shift hours: 8:00 AM - 4:30 PM Shift days: Monday-Friday Weekend requirement: Every 3rd Saturday Coverage area includes Wabash, Huntington, and Grant County, with the ability to support Blackford, Delaware, and Madison County as needed May require availability on observed holidays
