Minimum Requirements:
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.
- Active Washington State Physical Therapy license required to start the position.
- BLS certification from AHA or ARC may be required.
- Experience providing home health physical therapy preferred.
- Experience with OASIS documentation and ICD-10 coding.
- Ability to take initial patient history, perform evaluations, and develop plan of care under the direction of the Director of Clinical Services.
- Experience directing and supervising PTAs and Home Health Aides as assigned.
- Valid driver’s license and ability to travel within the greater Seattle area (coverage excludes west and south Seattle).

Responsibilities:
- Provide physician orders and implement plan of care under the direction and supervision of the Director of Clinical Services.
- Work with other home care personnel in interdisciplinary patient care planning.
- Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
- Take initial patient history and complete initial evaluations.
- Consult with physicians regarding changes in treatment or condition.
- Instruct patients and family in home programs and activities of daily living.
- Participate in Quality Assessment and Performance Improvement activities as assigned.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and documentation of significant change in condition.
- Utilize appropriate ICD-10 codes for documentation.
