Key Responsibilities
- Develop and update individualized care plans
- Provide physical therapy services per physician’s plan of care
- Assess functional status, mobility, and home environment
- Educate patients and families on therapy techniques and equipment
- Maintain accurate documentation and communicate progress to physicians
Requirements
- Licensed Physical Therapist in the State of Nevada
- Degree from a CAPTE-accredited program; passed state-approved exam
- Minimum 2 years’ experience (home health preferred)
- Current CPR certification
- Valid driver’s license, insurance, and reliable transportation
