LMSW Case Manager, Inpatient Social Work

Kaiser Permanente is seeking an LMSW Case Manager, Social Work – Inpatient II to support patients at our Capitol Hill facility in Washington, DC. In this role, you’ll manage complex inpatient cases, conduct psychosocial assessments, develop discharge and care plans, and coordinate services across interdisciplinary teams. You will leverage modern healthcare IT systems to document care, track outcomes, and streamline communication. You’ll collaborate closely with physicians, nurses, and community providers, advocate for patient needs, and ensure safe transitions of care while complying with regulatory and organizational standards.
