What You’ll Do
As a Behavioral Health Registered Nurse, you will:
Assess patients’ physical, behavioral, psychosocial, and environmental needs.
Develop, coordinate, implement, and evaluate individualized plans of care.
Manage an assigned caseload and complete initial and ongoing nursing assessments.
Monitor patients for changes in condition, safety concerns, and barriers to treatment.
Communicate important updates to physicians, clinical leadership, caregivers, and other members of the care team.
Educate patients and caregivers about medications, symptom management, safety, and treatment goals.
Update plans of care based on patients’ changing needs and progress.
Coordinate services with an interdisciplinary team to support positive patient outcomes.
Supervise and document care provided by LPNs and behavioral health aides when applicable.
Maintain accurate and timely clinical documentation.
What You’ll Need
Current, unrestricted RN license in the state of practice
Associate or bachelor’s degree in nursing
At least one year of clinical nursing experience
Behavioral health, home health, or hospice experience preferred
Homecare HomeBase experience preferred
Dependable vehicle, valid driver’s license, and current automobile insurance
Willingness to travel throughout the assigned territory
Ability to travel up to 50%
Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
