Cardiac Rehab Specialist II

Intermountain Health is seeking a Cardiac Rehab Specialist II to deliver evidence-based cardiac rehabilitation services for patients recovering from cardiac events and procedures. In this role, you will assess cardiovascular risk, develop individualized exercise and education plans, monitor patients’ responses using telemetry and vital-sign assessment, and provide lifestyle counseling on nutrition, activity, and risk-factor reduction. You’ll collaborate closely with physicians, nurses, and allied health professionals, document progress in the EMR, support quality and safety initiatives, and help patients and families build confidence in managing heart health in a supportive, team-based environment.
