We are building a Program of All-Inclusive Care for the Elderly (PACE) to bring life-changing, culturally competent care to low-income seniors—supporting them as they age safely and with dignity, right in their communities. Based on the PACE model, SpringLight Health will offer coordinated medical care, transportation, meals, social activities, medication management, and caregiver support—all tailored to each individual’s needs.

The PACE Dietitian Manager is responsible for the medical nutrition program for PACE participants.
The PACE Dietitian Manager is a member of the PACE Interdisciplinary Team (IDT) and is responsible for completing nutrition assessments for new PACE participants and nutrition reassessments on existing PACE participants as part of the PACE IDT care planning process.
The PACE Dietitian Manager is responsible for assessing participants' nutritional and dietary needs and incorporating industry best practices to implement appropriate nutritional and dietary interventions for PACE participants in collaboration with the IDT.
The PACE Dietitian Manager is responsible for planning food services at the PACE Center and ensuring participant food preferences are considered in PACE Center menu planning.
The PACE Dietitian Manager is responsible for providing nutrition and diet education to participants, caregivers, and PACE staff.
