HarmonyCares is seeking a Licensed Clinical Social Worker to provide in-home psychosocial assessments, counseling, and care coordination for medically complex, home-limited patients. In this role, you will support patients and families in navigating chronic illness, advance care planning, and community resources while collaborating closely with physicians, nurses, and care managers. You will conduct home and virtual visits, develop individualized care plans, document in the EHR, and help reduce avoidable hospitalizations.
