CenterWell Home Health seeks a Home Health RN Clinical Manager to lead in-home clinical operations for seniors and adults with chronic conditions. In this role, you’ll oversee a team of nurses and clinicians, coordinate patient care plans, ensure regulatory and quality standards, and support safe, effective transitions of care. You’ll manage caseloads, review documentation, coach and mentor staff, and collaborate with an interdisciplinary team to deliver personalized, patient-first home care that promotes independence and optimal outcomes.
