In-Home Nurse Practitioner

Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to deliver comprehensive, value-based care assessments in patients’ homes. You will perform detailed health evaluations, manage chronic conditions, reconcile medications, and coordinate with primary care and specialists to close care gaps. Using modern technology, EHRs, and data-driven insights, you’ll identify risks, support care plans, and improve outcomes for vulnerable populations.
