Full job description

St. David's Medical Center is seeking a compassionate Home Health Physical Therapist Assistant to deliver rehabilitative care to patients in their homes. Working closely with a supervising PT, you’ll implement treatment plans, provide therapeutic exercises, gait and balance training, and patient/family education to promote mobility and safety. You’ll document care, monitor progress, and collaborate with an interdisciplinary team.
