Hospice Medical Social Worker

Optimal Care is seeking a compassionate Hospice Medical Social Worker to support patients and families facing serious illness and end-of-life needs. In this role, you will complete psychosocial assessments, develop individualized care plans, and provide counseling, crisis intervention, and grief support. You will connect families with community resources, guide advance care planning, and advocate for patient preferences. Working closely with an interdisciplinary hospice team, you will document care, attend team conferences, make home and facility visits, and ensure dignified, patient-centered support.
