CenterWell Home Health, part of Humana Inc., is seeking a Home Health Registered Nurse to deliver compassionate, skilled nursing care in patients’ homes. In this role, you will assess patient needs, develop and manage individualized care plans, administer medications and treatments, and educate patients and caregivers to promote safety and independence. You’ll collaborate closely with an interdisciplinary team, use innovative mobile and electronic tools, and help seniors and adults with chronic conditions manage illness and avoid hospitalizations while living more independently at home.
