Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Ohio PT license
• Home Care experience
• PT certification

Assignment Details

• Shift hours: 8:00 PM - 4:00 AM
• Shift days: Monday through Friday
• Schedule: 8 hours per day, 5 days per week
• Coverage area includes Lucas County and surrounding areas
• Mileage is reimbursed between visits, from first patient to last patient
• Productivity expectation: 30 points
• Primary responsibility: Case Manager
• Dress code: Blue/Black

Location Highlights

Holland, Ohio offers convenient access to both suburban amenities and outdoor recreation. The community sits about 10 miles southwest of downtown Toledo and provides quick access to the Toledo Museum of Art and the Toledo Zoo. Outdoor options include Maumee Bay State Park, about 25 miles away on Lake Erie, and Wildwood Preserve Metropark, roughly 8 miles away with trails and wooded areas. Travelers in the area can also reach Oak Openings Preserve Metropark in about 20 minutes, a regional destination known for hiking, biking, and rare oak savanna habitat.
