Full job description
FOX Rehabilitation seeks a Physical Therapist to provide evidence-based geriatric care in patients’ homes and senior living communities. Evaluate, develop, and implement individualized treatment plans focused on function, fall prevention, and improved quality of life. Collaborate with an interdisciplinary team, educate patients and caregivers, and document care using EMR. Enjoy autonomy, strong clinical support, mentorship, and ongoing education in a clinician-founded, mission-driven practice dedicated to older adults.
