Now Hiring: Lead Case Manager

We’re looking for a Lead Case Manager to join our team and play a vital role in providing community-based support to foster youth and their families.

Lead Case Manager Responsibilities include but are not limited to:

  • Travel to provide in-home and community-based services to youth and families
  • Manage a caseload of foster youth and support foster homes
  • Coordinate services with schools, courts, medical providers, and community partners
  • Conduct assessments and assist in developing individualized service and treatment plans
  • Support family reunification and placement stability efforts
  • Maintain timely and accurate documentation of visits, services, and case progress
  • Build and maintain strong relationships with youth, families, and stakeholders
  • Participate in team meetings, court involvement, and on-call rotation as needed

Lead Case Manager Qualifications include but are not limited to:

  • Bachelor’s Degree in Social Work, Counseling, Psychology, Sociology, or related human services field
  • LSW or LPC is required
  • Minimum 2 years of relevant experience preferred (child welfare or working with youth strongly preferred)
  • Strong communication, organizational, and problem-solving skills
  • Ability to work independently and manage a flexible schedule
  • Valid driver’s license, reliable transportation, and ability to travel daily
