Minimum Qualifications
- Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Indiana Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Home health experience required.
- Experience with HCHB (Homecare Homebase) required.
- Experience performing SOC, ROC evaluations, recertifications, and revisits required.
Preferred Qualifications
- Previous travel therapy experience preferred.
- Experience working in St. Joseph and Marshall County home health markets preferred.

- Desired territory: St. Joseph and Marshall County.
- Key duties include SOC, ROC evaluations, recerts, and revisits.
- Expected hours/points: 27 points per week.
- Candidate must be able to work in a home health environment and complete documentation in HCHB.
