Full job description
The Clinical Hematology Pharmacist Specialist provides advanced medication management for patients with benign hematologic disorders in an ambulatory clinic setting. Working closely with hematologists and the care team, this pharmacist designs, initiates, and adjusts evidence-based therapies; monitors efficacy, safety, labs, and interactions; and educates patients and caregivers. The role also supports malignant hematology services, leads quality and safety initiatives, and contributes to guidelines, research, and protocol development.
