Hospice Medical Director

Heartland Hospice, part of ProMedica, seeks a Hospice Medical Director to lead our interdisciplinary hospice and palliative care program. The Medical Director provides clinical leadership, supervises hospice physicians and APRNs, and ensures high-quality, patient-centered end-of-life care across home and facility settings. Responsibilities include certifying and recertifying hospice eligibility, managing complex pain and symptom needs, collaborating with nurses, social workers, chaplains, and therapists, and guiding difficult goals-of-care and advance care planning conversations with patients and families. The Medical Director also supports quality improvement, regulatory compliance, staff education, and community/physician outreach while modeling our mission of compassion, dignity, and respect.
