How you’ll make an impact in this role

• Lead multidisciplinary care planning from admission to discharge, integrating patient preferences, addressing social determinants of health, and arranging post-acute services to ensure safe transitions.

• Evaluate clinical criteria (InterQual/MCG) continuously to ensure accurate level-of-care assignments, collaborate with payers for timely authorization, and prevent avoidable delay days.

• Serve as a clinical liaison across care teams, educating staff on thorough medical record documentation while maintaining ongoing dialogue with supervisors and care transition teams

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
• Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
