Home Health Speech Language Pathologist

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to provide compassionate, in-home therapy to seniors and adults with chronic conditions. In this per diem role, you will conduct speech, language, and swallowing evaluations, develop individualized treatment plans, and deliver evidence-based interventions that help patients communicate better and live more independently at home. You’ll collaborate closely with an interdisciplinary team, document care in our electronic systems, and educate patients and caregivers on exercises and safety strategies.
