Responsibilities

  • Provide comprehensive primary care to patients across the lifespan
  • Conduct physicals, wellness visits, acute visits and follow-up care
  • Evaluate, diagnose and manage acute and chronic conditions
  • Provide preventive health counseling and patient education
  • Coordinate care with specialists and other healthcare team members
  • Utilize telemedicine when appropriate
  • Maintain accurate and timely electronic medical records

  • Current New York State Nurse Practitioner or Physician Assistant license
  • Graduate of an accredited NP or PA program
  • Family medicine or primary care experience preferred
  • Strong clinical, communication and interpersonal skills
  • Commitment to patient-centered, compassionate care
