CenterWell Home Health, part of Humana Inc., seeks a Home Health Psych Registered Nurse to provide in-home psychiatric nursing care for seniors and adults with chronic conditions. In this role, you’ll complete psych assessments, develop individualized care plans, administer and manage medications, and coordinate with an interdisciplinary team to support safety and stability at home. You’ll educate patients and families, monitor behavioral changes, and intervene in crises while documenting in EHR.
