American Traveler is seeking a Physical Therapist for a Home Health assignment requiring 1+ year of skilled home health experience and BLS certification. Details

Home health setting serving patients throughout Medford, OR and surrounding areas 1:1 patient ratio with up to 6 patient visits per day Standard productivity expectation of 30 points Day shift schedule, 5x8 hours from 8:00 AM to 4:30 PM No weekend requirements No on-call requirements No floating between like units required Uses Epic/Oasis for charting Occasional maximum drive radius of 50+ miles between patient visits Requirements

Minimum 1 year of skilled home health experience required BLS certification required Oasis charting experience required Epic EMR experience required Must have reliable personal transportation Primary series COVID vaccination plus one booster required License required if applicable Additional Information

Any color scrubs accepted Free parking available Mileage reimbursement provided at IRS standard rate System groups clients to help minimize drive and travel time between visits First-time travelers will not be considered for this role Facility may cancel up to one shift per pay period
