LPN Med Care Manager

Sunrise Senior Living seeks a compassionate LPN Med Care Manager to provide high-quality medication management and clinical support for residents in our assisted living and memory care community. Responsibilities include safely administering medications and treatments, monitoring resident health, updating care plans, documenting in accordance with regulations, and communicating changes to physicians, families, and the care team. You will coach and support care staff, model Sunrise’s resident-centered approach, and help create a safe, dignified, inclusive environment that supports residents’ independence and quality of life.
