In this role, you will assess functional abilities, create individualized treatment plans, and provide evidence-based interventions to help patients maximize independence in daily activities. You will collaborate closely with an interdisciplinary healthcare team, maintain accurate EMR documentation, and educate patients and families on home programs and adaptive equipment.
